Appendix № 9

List of attestation documents to be sent to the HAC by the Academic Council awarding academic degrees:

• Follow-up letter on the letterhead of the institution signed by the chairman of the academic council (chairman of the meeting) awarding academic degrees.
• Objective-informative reference about the applicant.
• The original transcript of the meeting of the Academic Council, signed by the Chairman of the Academic Council (Chairman) and the Academic Secretary of the Academic Council (the transcript of the members of the Academic Council is attached to the transcript).
• Reviews of official opponents and leading organization.
• Copies of announcements on the defense of the dissertation on the website of the HAC or in the magazine “Bulletin of the HAC” and in the central newspaper of the republic.
• Documents proving that the results of the dissertation are used in practice;
• A copy of the applicant’s passport.
• Documents confirming the activities of the applicant and the results of attestation (a copy of the document on higher education; a copy of the diploma of the candidate of Doctor of Philosophy (PhD) or other equivalent degree for the applicant for the degree of Doctor of Science (DSc); originals of certificates; written consent of co-authors for full or partial inclusion of co-authored scientific works in the dissertation).
• One copy of the dissertation and two copies of the dissertation abstract and copies of the main scientific works of the applicant.
• Abstract of a PhD or other equivalent dissertation for a PhD or equivalent.
• SD-disk with electronic version of the dissertation, dissertation abstract and objective reference about the applicant.

The list of the documents submitted by the Academic Council or the institution where the dissertation was conducted on the establishment of a one-time scientific council or submitted by the candidate to the Academic Council awarding academic degrees:

• Application of the institution where the dissertation was completed for the defense or the establishment of a one-time scientific council.
• An SD-disk with the objective informative reference of the applicant and its electronic form.
• A copy of the applicant’s higher education diploma.
• A copy of a PhD or equivalent degree (for a DSc candidate).
• Certificates of passing the qualifying exams (for applicants for the degree of Doctor of Philosophy (PhD)).
• The conclusion of the institution where the dissertation was completed.
• Conclusions of the organizations where the dissertation was discussed (if any).
• List of scientific works of the candidate, approved by the institution where the dissertation was performed, copies of the main scientific works.
• Review of the scientific advisor.
• The abstract of the dissertation by the candidate of PhD or other equivalent degree for the degree of Doctor of Science (DSc).
• Written consent of the co-authors to include all or part of the co-authored research work in the dissertation.
• Dissertation and dissertation abstract.

The list of the documents submitted for publication and announcement of the dissertation defense on the website of the Higher Attestation Commission:

• Referral letter signed by the chairman of the academic council awarding degrees.
• An SD-disc with an objective reference about the applicant and its electronic form.
• An SD-disk with the text of the announcement of the dissertation defense and its electronic form.
• Copies of documents submitted for the implementation of the scientific results of the dissertation.
• Manuscript of the dissertation abstract and SD-disk with electronic version of the dissertation and abstract.
